Tragedy of six-week-old baby Olivia

A MUCH-LOVED six- week-old baby died after a plastic nappy bag somehow covered her face while she slept, an inquest heard.

The bag had been left at the other end of Olivia Mae Chandler's cot when she fell asleep the evening before, Gloucestershire Assistant Deputy Coroner Tom Osborne was told.

But in the morning it was found covering the baby's face and inside her mouth.

The little girl, who lived with her parents and two siblings in Beaufort Road, Gloucester, was found with the bag over her face on the morning of March 15 last year.

Her father David carried out CPR and paramedic Gary Mills was on the scene a minute after receiving the call.

He continued the CPR, but quickly realised further efforts were futile.

Detective Sergeant Nigel Hatton carried out an intensive investigation into the circumstances surrounding the death.

He interviewed all the other members of the family and found Olivia Mae was a "much-loved and wanted little girl" and the tragedy had affected them all deeply.

"It is impossible to say how the bag came to cover the baby's face," he said. "But I believe it contributed to her death.

"The other daughter who found her said it felt like the bag had been inside her mouth as she pulled it away.

"The bag had been left at the other end of the cot, well away from the baby."

Dr Richard Jones, consultant professor of pathology at Cardiff University, carried out a post mortem and said bleeding he found in the lungs could have been caused by airway obstruction, but was also found in the infant deaths for which no cause could be found.

He agreed with the coroner the most likely cause of death was airway obstruction by the nappy bag.

Mr Osborne recorded a verdict of accidental death.
